Carefeed is seeking a Clinical Implementation Specialist – Referral Management to help senior living and long-term care clients optimize central intake, prior authorizations, and concurrent reviews. You will bridge clinical operations with technology, train teams, and establish repeatable workflows that drive adoption.

The role emphasizes implementation, client education, and workflow improvement rather than direct patient care, with remote delivery across the USA.

Qualifications

• Five+ years of experience in skilled nursing, short-term rehab, or closely related post-acute care operations.
• Hands-on responsibility for central intake, prior authorization, and concurrent or continued-stay review.
• Experience coordinating referrals, verifying insurance/benefits, and tracking authorization requirements.

Responsibilities

• Lead client discovery and workflow planning across intake, referral review, verification, and authorization.
• Translate operational needs into module usage and handoffs with EHRs and payer portals.
• Deliver role-based training for central intake, admissions, nursing, MDS, therapy, and business offices.
• Manage implementation milestones, go-live readiness, and handoff to support teams.
• Serve as an internal SME on skilled nursing referrals, authorization, and managed care workflows.
• Create training materials, workflows, readiness checklists, SOPs for scalable delivery.
• Collaborate with Product, Client Experience, and Support to address workflow challenges.
