Referral Specialist for NHC - PRN

NHC – National HealthCare Corporation is looking for a Referral Specialist to join our team. The Referral Specialist executes referral management process with all referral sources and all NHC Center locations.

Our goal is to provide a full range of extended care services, designed to maximize the well-being and independence of patients of all ages.

We transition patients from the acute setting to post-acute care, 34 Skilled Nursing Facilities (SNFs) and 19 Home Health Care agencies. We are located Pulaski TN adjacent to NHC HealthCare Pulaski.

As a Referral Specialist Skilled Nursing Facility, your duties will include managing multiple referral platforms, summarise patient referrals using medical terminology, collaborating between accepting SNF and referral source, and ensuring internal and external customers’ needs are met. You will be responsible for accurately completing referral workflows, data entry of referrals, and conversions of referrals to admissions.

Preferred Qualifications
