Carefeed is hiring a Clinical Implementation Specialist – Referral Management to bridge clinical operations and technology. You will lead discovery, configure workflows, train teams, and ensure smooth adoption of the Referral Management module for skilled nursing and rehab settings.

The role emphasizes technology implementation, education, and workflow improvement with no direct patient care. Remote work available within the USA.

Competenze

• 5+ years in skilled nursing or rehab operations with central intake and authorization experience.
• Experience coordinating referrals and payer communication.
• Strong knowledge of clinical documentation and MDS/PDPM.
• Ability to train others and lead workflow changes.

Mansioni

• Lead client discovery and plan workflows for referral management.
• Translate needs into module use and handoffs between systems.
• Deliver role-based training for multiple care teams.
• Manage go-live and transition with milestones and actions.
• Develop repeatable training resources and SOPs.
• Collaborate with Product/Support to improve workflows.

Conoscenze

5+ years experience in skilled nursing

Referral coordination experience

Payer communication experience

Clinical documentation knowledge

MDS and PDPM understanding

Strong training & facilitation
