As a Referral Specialist skilled nursing facility, your duties will include managing multiple referral platforms, summarizing patient referrals using medical terminology, collaborating between accepting SNFs and referral sources, and ensuring internal and external customers’ needs are met. You will be responsible for accurately completing referral workflows, data entry of referrals, and converting referrals to admissions.

Preferred Qualifications:

• Experience: 1–2 years in a healthcare setting including, but not limited to, case management, nursing, coding, billing, medical terminology, etc.
