Job summary

Advocate Health in Charlotte, NC is seeking a part-time Social Worker for the Hospital at Home - Virtual Health program. You will interview patients and their social support system to complete psychosocial evaluations, provide counseling, and help develop a care plan with a multidisciplinary team.

The role requires a Master’s in Social Work (LCSW preferred), at least 1 year of hospital or health setting experience, and familiarity with data management tools.

Qualifications

• Master's degree in Social Work from a program accredited by the council in Social Work Education required.
• LCSW preferred.
• At least 1 year professional hospital or health-related setting preferred.
• Expertise with Data Management Tools.
• Adherence to National Association of Social Workers Code of Ethics.

Responsibilities

• Interviews patient and social support system to complete psychosocial evaluation.
• Provides counseling, support, and education to patient and social support system.
• Collaborates with multidisciplinary team to implement plan of care.
• Serves as a resource to staff regarding psychosocial aspects of illness.
• Intervenes in crisis situations using appropriate crisis intervention techniques.
• Completes psychosocial assessments for post-hospital care.
