Qualifications

• Master's degree in Social Work required.
• LCSW preferred; if working in SC, license requirements must be met.
• At least 1 year of hospital or health care setting experience preferred.

Responsibilities

• Interviews patient and family to complete psychosocial evaluation.
• Provides counseling and education to patient and social support network.
• Assists in planning and implementing a multidisciplinary plan of care.
• Assesses barriers to post-hospital care and facilitates referrals.
• Documents assessments and maintains case records per departmental standards.
