Advocate Health in Charlotte, NC seeks a part‑time social worker to support the GCMG Adult Hospital at Home – Virtual Health program. You will conduct psychosocial evaluations, provide counseling, and collaborate with a multidisciplinary team to plan care across the continuum.

Requirements include a Master’s in Social Work; LCSW preferred; at least 1 year hospital or health‑care experience; strong data management and ethics knowledge.

Competenze

• Master's degree in Social Work required; LCSW preferred.
• At least 1 year professional hospital or health setting experience.
• Experience with Data Management Tools.
• Adherence to NASW Code of Ethics.

Mansioni

• Interviews patient and social support to complete psychosocial evaluation.
