Job summary

Advocate Health in North Carolina is seeking a Home Medical Social Worker (PRN) to support patients and families, assess psychosocial needs, provide counseling, and coordinate resources for post-hospital care.

The role collaborates with clinical teams to develop care plans, addresses crises, and documents in medical records. MSW required; LCSW preferred; supervision and travel within the service area may be needed.

Qualifications

• Master's degree in Social Work is required.
• LCSW preferred.
• At least 1 year hospital or health setting experience is preferred.

Responsibilities

• Interviews patient and socialSupport system to complete psychosocial evaluation.
• Provides counseling and education to patient and social support system.
• Collaborates with multidisciplinary team to plan care addressing psychosocial needs.
• Maintains case records and documents in medical records.
• Assesses appropriate level of care and connects to community resources.
• Reports suspected abuse/neglect and coordinates with Department of Social Services.
