Job summary

IntellaTriage is seeking a compassionate registered nurse to join as a remote hospice triage RN. You will provide after-hours phone triage, assess patient needs, and coordinate care with on-call teams to ensure timely interventions. A multistate RN license is required, with hospice or end-of-life care experience preferred.

Qualifications

• Active multistate RN license with hospice or palliative care experience preferred.
• Comfort with technology and EMR documentation during calls.
• Ability to type notes quickly while multitasking in a fast-paced remote environment.
• Strong communication and critical thinking to coordinate timely patient care.

Responsibilities

• Provide telephone triage for hospice patients and families.
• Assess patient conditions and determine appropriate next steps.
• Collaborate with on-call teams to coordinate care and resources.
• Accurately document all communications and interventions.
• Maintain a calm and professional demeanor during urgent calls.
