Job summary

IntellaTriage is seeking a compassionate registered nurse (RN) to join our remote after-hours triage team. You will provide telephone triage for hospice patients and families, assess conditions, and coordinate care with on-call teams from a home office.

Active multistate RN license is required; hospice or end-of-life care experience is preferred. You’ll work independently from home with reliable internet, strong communication, and the ability to type notes quickly during high-volume calls.

• Hospice, palliative, or end-of-life care experience preferred
• Comfortable with technology and EMR for documentation
• Ability to type notes quickly during fast-paced calls

Responsibilities

• Provide telephone triage for hospice patients and families
• Assess patient conditions and determine appropriate next steps
• Coordinate care with on-call teams to ensure timely interventions
• Accurately document all communications and interventions
• Maintain a calm, professional demeanor on urgent calls
