Job summary

TRU Community Care is seeking a phone triage nurse to join our afterhours triage team. This PRN role handles calls from patients, assesses needs, and works with visit nurses to determine the best care plan. Must reside in Colorado, Florida, or Georgia and be able to work remotely.

This role requires strong clinical skills, excellent communication, and adherence to HIPAA and agency protocols, with occasional in-person training near Lafayette, CO.

Qualifications

• Active RN license in Colorado or compact license.
• Residence in Colorado, Florida, or Georgia.
• Minimum 1 year RN experience; hospice/home health/PACE preferred.
• Pass agency background checks and drug screening.

Responsibilities

• Answer triage calls and delegate tasks to the care team.
• Monitor triage calls within agency time limits.
• Evaluate patients' health care needs using triage protocols.
• Obtain input from providers to determine best action for patient care.
• Direct to emergency room, urgent care, or arrange in-home nursing as needed.
• Apply triage protocols appropriately.
