Job summary

TRU Community Care is seeking a Phone Triage Registered Nurse to handle after-hours calls, collaborating with visit nurses on patient needs. This PRN role offers mostly remote work with a requirement to reside in Colorado, Florida, or Georgia and occasional in-person trainings near Lafayette, CO.

You will use triage protocols to assess patients, determine the best care actions, and guide patients to appropriate services such as home nursing or urgent care as needed.

Qualifications

• Must possess a current RN license in the state of Colorado or compact nursing license.
• Must reside in Colorado, Florida, or Georgia.

Responsibilities

• Answers calls to the afterhours triage line and supports the Care Team.
• Monitors triage calls and adheres to time limits.
• Assesses patient needs using triage protocols and coordinates with the provider.
• Directs patients to appropriate care settings as needed (ER, urgent care, home health).
• Uses established triage protocols to determine the best course of action.
