TRU Community Care is seeking a Phone Triage RN to support afterhours triage by handling calls from patients and coordinating with visit nurses for patient needs. The role is mostly remote; you must reside in Colorado, Florida, or Georgia.

A minimum of one year RN experience, hospice/home health or PACE experience preferred, with HIPAA compliance and reliable internet required. Compensation ranges from $42.34 to $50.81 per hour, PRN shifts with a minimum of two shifts per month.

Qualifications

• Must have a secure HIPAA-compliant workspace with reliable internet access.
• Must possess a current RN license in the state of Colorado or compact licensing.
• Must reside in Colorado, Florida, or Georgia.

• Must pass background checks and health screenings.

Responsibilities

• Answers triage calls and delegates tasks for the care team.
• Monitors triage response within agency time limits.
• Evaluates patient needs using triage protocols and guidelines.
• Obtains input from providers to determine best clinical actions.
• Directs patients to ER, urgent care, or arranges in-home nursing visits as needed.
• Utilizes triage protocols appropriately.

Title: Hospice Phone Triage RNPay: $42.34 to $50.81 per hourSchedule: PRN, minimum 2 shifts per month. Typically 12 hours each.Location: Mostly remote, must reside within Colorado, Florida, or Georgia . Will require occasional training near, and there will be hiring preference for, the Lafayette, CO area.

JOB SUMMARYTRU Community Care is looking for a phone triage nurse. This nurse will work with our afterhours and triage team, taking in calls from patients and collaborating with visit nurses on patient needs as necessary.

ESSENTIAL DUTIES

• The Triage Registered Nurse performs in a supportive role to the Care Team by answering calls coming into the agency, with response to the telephone call by delegating tasks.
• Monitors and responds to triage calls within the agency specific time limits.
• Evaluates and assesses patients' health care needs based on patient's signs and symptoms utilizing established triage protocols and guidelines.
• Obtains appropriate input and direction from the provider as needed to determine the best clinical course of action and recommendations for specific patient health care needs.
• May direct them to emergency room, urgent care, provide home care advice, or arrange for an in-home nursing visit.
• Utilize triage protocols appropriately.

EDUCATION & REQUIREMENTS

• Must have a secure HIPAA-compliant workspace with a door, no background noise or interruptions, and a reliable wired high-speed internet connection with a minimum of 150Mpbs download and 100Mbps upload capabilities.
• Must possess a current RN license in the state of Colorado or compact nursing license
• Must reside in Colorado, Florida, or Georgia
• Must have a minimum of one year RN experience, with hospice, home health, or PACE experience preferred
• Must successfully pass agency required background checks and drug screening
• Must comply with agency health screening and immunization requirements
