Job summary

TRU Community Care is seeking a Phone Triage RN to support afterhours triage. You will handle patient calls and collaborate with visit nurses to determine the safest and most appropriate care plan.

This PRN role requires an active RN license in Colorado or compact license, residency in Colorado, Florida, or Georgia, and at least one year of RN experience (hospice/home health/PACE preferred). A secure HIPAA workspace, reliable internet, and the ability to pass background checks and drug screening

Qualifications

• Must have a HIPAA-compliant workspace with minimum noise.
• Must possess a current RN license in Colorado or a compact license.
• Must reside in Colorado, Florida, or Georgia.
• Minimum of one year RN experience; hospice/home health/PACE preferred.
• Must pass agency background checks and drug screening.

Responsibilities

• Answer calls into the agency and triage patient needs.
• Collaborate with visit nurses to determine the best care plan.
• Evaluate and respond to triage calls within set time limits.
• Provide guidance on next steps, including referrals as needed.
• Adhere to established triage protocols.
