Job summary

Mount Sinai Morningside is seeking a Care Coordinator to support logistical patient care activities for the Mount Sinai at Home program. You will coordinate with clinicians, vendors, and patients to manage orders, schedules, and communications, while maintaining privacy and safety standards.

The role requires healthcare experience, EMR proficiency, and strong organizational skills. A health-related degree is required, with Spanish language skills preferred for enhanced patient communication.

Qualifications

• Experience coordinating care for patients and families with clinical staff.
• Ability to identify data elements to support quality improvement.
• Experience drafting organizational workflows and protocols.
• Experience with EMR; strong téléphonic and coordination skills.
• Proven ability to measure and communicate program successes.

Responsibilities

• Assess and address complex care needs as part of an interdisciplinary team.
• Support admission and home-based census operations and coordinate care orders.
• Provide administrative support for tracking care activities (vendor liaison, equipment pickup, scheduling, discharge confirmations).
• Communicate plan of care and assessments to front line teams.
• Participate in daily/weekly huddles with care teams.
• Meet with leadership monthly to discuss quality metrics and workflow improvements.
• Maintain operational database with relevant care metrics.
• Serve as liaison between physicians, clinical staff, and vendors.
• Set priorities and exercise judgment in difficult situations.
• Monitor Epic orders and respond to clinical team calls.
• Ensure HIPAA compliance and protect patient privacy.
• Maintain records of interventions and safety events.
• Submit utilization and assignment reports each shift.
• Maintain attendance/punctuality per department policy.
• Perform other duties as assigned.
