Job summary

Mount Sinai Health System is seeking an on-site Registered Nurse to identify, evaluate, and enroll patients into Mount Sinai at Home programs. You will work with ED and hospital teams across locations, coordinating admissions and home safety/eligibility from an office setting by phone and email.

Qualifications

• Graduation from an accredited Nursing program with RN licensure in New York State.
• Bachelor’s Degree in Nursing (BSN) or pursuing BSN with graduation date within the year; ADN acceptable with approval by CNO.
• Current, unencumbered license to practice as an RN in New York and Basic Life Support (BCLS) certification.
• 1+ year of RN experience; ability to work both day and evening shifts, rotating holidays, and on-call.

Responsibilities

• Identify and evaluate patients for Mount Sinai at Home program eligibility.
• Coordinate admissions and communicate with ED/hospital teams to ensure proper orders and home safety.
• Maintain documentation and coordinate services ordered for physicians to move care plans forward.
