Job summary

A-Line Staffing Solutions LLC is seeking a Health Navigator (Remote) to support members in case management and disease management programs from a California-based remote position. You will conduct outreach, educate members on benefits, and coordinate care to improve continuity of care.

Ideal candidates have 3+ years in healthcare or healthcare customer service, strong documentation skills, and the ability to work independently in a metrics-driven environment.

Qualifications

• 3+ years experience in healthcare and/or healthcare customer service (managed care/health plan preferred).
• 1+ year experience supporting care coordination or discharge planning preferred.
• Strong medical/insurance terminology knowledge and member-focused communication style.
• Strong typing/data entry and documentation accuracy; comfort with phone-based work.
• Proficient with Microsoft Office (Word/Excel/Outlook) and web-based systems.
• Organized, detail-oriented, and able to manage a caseload independently.
• TB clearance required prior to or within seven (7) days of hire (as applicable).

Responsibilities

• Conduct member outreach, assess needs, and support care coordination across the care continuum
• Educate members on benefits, eligibility, referrals, and service utilization
• Coordinate with providers, caregivers, discharge planners, and internal teams to support continuity of care
• Identify and connect members to community and support resources as appropriate
• Maintain accurate documentation of calls, case activities, and follow-ups in care management systems
• Meet productivity expectations (call volume/documentation volume) and quality audit standards
• Escalate cases appropriately to clinical support (RN/ECM team) when needed
