Job summary

A-Line Staffing is seeking a Health Navigator in Alameda, CA to help members access healthcare, behavioral health, and community resources. The role emphasizes care coordination, education on health plan benefits, and scheduling of appointments.

You will work with providers and case managers, manage a caseload, document interactions, and ensure HIPAA compliance while meeting productivity and quality standards for a temp-to-perm opportunity.

Qualifications

• Bachelor's degree in healthcare or equivalent professional healthcare experience.
• Three years of healthcare or healthcare-related customer service experience preferred.
• One year of care coordination, care planning, or discharge planning experience preferred.
• Fluency in English, including strong written communication.
• Proficiency with Windows and Microsoft Office.

Responsibilities

• Contact and assess members who may benefit from care management services.
• Coordinate care with members, families, physicians, hospitals, case managers, and community organizations.
• Explain health plan benefits, eligibility, services, and referral options.
• Help members schedule appointments and access healthcare and social services.
• Provide basic health education, self-management guidance, and community resource referrals.
• Document member interactions, care coordination activities, and follow-up actions accurately and promptly.
• Support Enhanced Care Management (ECM) members and coordinate with external ECM providers.
• Identify barriers to care and escalates clinical concerns to nurses or appropriate care management teams.
• Maintain an assigned caseload and meet productivity and quality standards.
• Follow HIPAA requirements, organizational policies, and applicable regulatory standards.
