Job summary

Acentra Health is seeking a Nurse Case Manager to join our team from a remote location in California. The role evaluates beneficiaries, develops care plans, and coordinates the healthcare team to meet CMSA principles and payer requirements.

Candidates must hold a CA RN license, have at least 3 years of clinical experience and 1+ year in case management review, and be able to work independently with strong communication and confidentiality.

Qualifications

• Bachelor's degree/BSN required or equivalent.
• Active, unrestricted RN license in CA.
• Minimum 3 years of clinical experience.
• Minimum 1 year of case management review experience.
• Must reside in CA and be able to work remotely from CA.

Responsibilities

• Assess, plan, implement, monitor, and evaluate an individualized care plan across the continuum of care.
• Coordinate with the health care team to ensure optimal beneficiary outcomes while adhering to payer requirements.
• Develop beneficiary-centered care plans and maintain confidentiality and compliance with HIPAA.
• Maintain accurate documentation and timely data entry into web-based systems.
• Pursue ongoing education and stay current with case management standards.
