A-Line Staffing Solutions LLC is seeking a Health Navigator (Remote) to support members in case management and disease management programs from a California-based remote position. You will conduct outreach, educate members on benefits, and coordinate care to improve continuity of care.

Ideal candidates have 3+ years in healthcare or healthcare customer service, strong documentation skills, and the ability to work independently in a metrics-driven environment.

Formación

• 3+ years experience in healthcare and/or healthcare customer service (managed care/health plan preferred).
• 1+ year experience supporting care coordination or discharge planning preferred.
• Strong medical/insurance terminology knowledge and member-focused communication style.
• Strong typing/data entry and documentation accuracy; comfort with phone-based work.
• Proficient with Microsoft Office (Word/Excel/Outlook) and web-based systems.
• Organized, detail-oriented, and able to manage a caseload independently.
• TB clearance required prior to or within seven (7) days of hire (as applicable).

• Meet productivity expectations (call volume/documentation volume) and quality audit standards
• Escalate cases appropriately to clinical support (RN/ECM team) when needed

Health Navigator (Remote) - MUST live in California to Qualify

Location: Remote (Alameda, CA / California) Schedule: Monday–Friday, 8:00am–5:00pm PT Pay: $42.00/hour

Summary

Hiring a Health Navigator for a remote care coordination role supporting members in case management and disease management programs. The Health Navigator provides ongoing outreach, benefits education, and care coordination support to help members access services, remove barriers, and improve continuity of care. This Health Navigator position is a strong fit for candidates with health plan/member services experience, strong documentation skills, and comfort working independently in a structured, metrics-driven environment.
