Job summary

Scene Health is seeking a Health Coach, Enrollment to enroll participants into a high-touch health program for medication adherence. The role involves outbound calls, texting, and video check-ins to understand patient needs, present Scene services, and guide enrollment with empathy.

The Health Coach will coordinate with care teams, manage patient information in Salesforce, and help participants navigate app setup and program participation while supporting overall health goals.

Qualifications

• 3+ years of experience in health education, patient engagement, or a related healthcare role.
• Background in health coaching, public health, or community health work.
• Strong interpersonal and empathetic communication skills.
• Experience conducting outreach via phone, SMS, and video communication.
• Ability to explain complex health concepts in a simple, motivating way.
• Proficiency with database systems and mobile applications.

Responsibilities

• Outreach to eligible program participants via phone, text, and email to introduce Scene services for enrollment.
• Identify patient needs and match them to Scene programs and insurance benefits.
• Guide patients through enrollment with motivational support and clear steps.
• Maintain accurate patient records in Salesforce.
• Overcome objections and technical barriers to ensure onboarding.
• Assist patients with technical setup, app downloads, and troubleshooting.
