Job summary

Scene Health is seeking a bilingual RN to engage patients through daily video check-ins, provide education and motivational coaching, and support adherence and care plans using asynchronous video tools.

The role focuses on overcoming social determinants of health barriers, coordinating with providers, and documenting interactions per regulatory standards, with remote shifts available in PR and the US. You will join a mission-driven team transforming medication adherence through digital health.

Qualifications

• Must be a legal resident of Puerto Rico.
• RN license in good standing with at least 5 years of nursing experience.
• 5 years of related experience in patient engagement, health education, or care coordination.
• Fluent in English and Spanish, reading/writing/speaking proficiently.

Responsibilities

• Conduct patient outreach and engagement through video messaging, consultations, calls, and texts to educate and enroll into the adherence program.
• Deliver personalized education, coaching, and adherence support via video communications.
• Provide coaching, case management, and health education to encourage adherence and lifestyle changes.
• Perform video-based medication reconciliation and ensure understanding of dosing and administration.
