Job summary

AdaptHealth is seeking a Care Coach to support patients across DME product lines, improve therapy adherence, and enhance health outcomes. You will coach patients, monitor device usage data, and document interactions in the EMR. You will collaborate with clinical, operational, and provider teams to resolve patient needs and ensure continuity of care.

The role requires strong communication, technical aptitude, and patient-centered care while complying with HIPAA and organizational standards.

Qualifications

• Motivational interviewing, behavioral coaching, or patient education preferred.
• Foundational knowledge of chronic conditions and related therapies.
• Empathetic, patient-centered support with active listening and conflict resolution.
• Familiarity with device compliance reporting tools, data analytics, and remote monitoring.
• Technical aptitude to understand device functionality and data interpretation.
• Strong attention to detail and written/verbal communication.
• Experience educating stakeholders on documentation and adherence standards.

Responsibilities

• Provide one-on-one mentoring and coaching to patients on device use.
• Educate patients on proper use, benefits, and maintenance of devices.
• Monitor adherence data via device software and apps; intervene as needed.
• Document patient interactions and device data in the EMR accurately.
• Coordinate follow-up with patients and healthcare providers to ensure care continuity.
• Collaborate with local branches, clinical staff and resupply teams to resolve needs.
• Ensure compliance with HIPAA and company confidentiality policies.
