Job summary

Scene Health is seeking a per-diem Registered Nurse (RN) to engage patients through daily video check-ins, provide education, motivation, and accountable adherence support. This remote role focuses on asynchronous video communication to guide patients toward better health outcomes and medication management.

Qualifications

• Must be a legal resident of Puerto Rico.
• RN license in good standing with at least 5 years of nursing experience.
• 5 years of related experience in patient engagement, health education, or care coordination.
• Fluent in English and Spanish (read, write, speak, understand).
• Strong background in chronic disease management and medication adherence.
• Excellent oral and written communication with a warm, empathetic approach.
• Proficient with EHR, mobile apps, and digital health platforms.
• Comfortable engaging patients via video, phone, and chat.
• Experience assessing social determinants of health.

Responsibilities

• Outreach and enroll patients in the adherence program via video messaging, consultations, calls, and texts.
• Deliver personalized education, coaching, and adherence support through video.
• Provide compassionate coaching, case management, and health education.
• Conduct video-based medication reconciliation and ensure proper dosing and administration.
• Review patient video submissions for adherence, side effects, and concerns.
• Create short educational and motivational videos for patient engagement.
• Assess patient needs and elevate cases when necessary.
• Guide on inhaler techniques, chronic disease management, and medication safety.
• Serve as clinical escalation point for non-clinical staff.
