CVS Health is seeking a Case Management Coordinator for Illinois with 40 hours weekly, primarily telework and substantial travel in Will, Kankakee and Grundy counties. You will support members by scheduling appointments, navigating benefits, and coordinating care with multidisciplinary teams.

The role requires a Bachelor's degree or higher in Behavioral Health or Human Services, and at least 2 years of related experience, including electronic documentation in Office and EHR systems.

Qualifications

• 2+ years in Behavioral Health, Social Services or related field.
• 2+ years of electronic documentation experience with Microsoft Office.

Responsibilities

• Evaluate members' needs and eligibility and propose case resolutions.
• Coordinate and monitor assigned care plan activities.
• Collaborate with case managers, Medical Directors and health programs.
• Provide coaching and support to empower informed health decisions.
