Qualifications

• 2+ years experience in Behavioral Health or Social Services.
• Willing to travel 50-75% of the time in Will, Kankakee and Grundy Counties and surrounding areas, Illinois.
• 2+ years of electronic documentation experience and Microsoft Office applications.

Responsibilities

• Evaluation of Members: assess needs/eligibility and plan care.
• Identify high risk factors and coordinate appropriate referrals.
• Coordinate and monitor care plan activities and progress.
• Collaborate with medical staff to improve care quality and outcomes.
• Escalate quality of care issues through established channels.
• Negotiate options to meet member benefits and needs.
• Motivate member engagement for healthier choices.
• Provide coaching and support for independent decision-making.
• Participate in team meetings and peer mentoring.
