Job summary

CVS Health is seeking a Case Management Coordinator to support members by coordinating care plans, scheduling appointments, and navigating benefits. This is a full-time remote role requiring 50-75% travel within DuPage, Kane, DeKalb, Kendall counties in Illinois.

You will collaborate with care managers and Medical Directors to improve outcomes, document care activities, and engage members in health decisions through coaching and motivational interviewing.

Qualifications

• 2+ years experience in Behavioral Health, Social Services or related field.
• Willing to travel 50-75% within Illinois.
• 2+ years of electronic documentation experience and MS Office proficiency.

Responsibilities

• Evaluate members' needs and eligibility and coordinate care plan activities.
• Identify high-risk factors and refer to clinical case management or crisis intervention.
• Coordinate and monitor care plan progress with team and providers.
