Qualifications

• Bachelor's degree or non-licensed master's in behavioral health or human services required.
• 2 years’ experience in behavioral health or related field.
• Excellent communication, analytical, and problem-solving skills.

Responsibilities

• Coordinate care plans and monitor progress.
• Identify high-risk factors and refer to clinical case management.
