Job summary

Community Care of North Carolina is seeking a Health Concierge who supports the Care Management team by documenting referrals, coordinating member outreach, and verifying eligibility. This role may work remotely within regions and may be on site at CCPN practices.

The Health Concierge will help address care gaps and facilitate health education and resource access. The position emphasizes collaboration, data entry accuracy, and adherence to privacy policies.

Qualifications

• High school diploma or GED required.
• 2-4 years minimum experience in a health care setting required.
• 2 or 4-year degree in health-related field preferred.

Responsibilities

• Receive and document referrals into Care Management platform.
• Outreach and scheduling of members for Care Managers.
• Verify eligibility and demographic information.
• Complete screenings as needed.
• Assist with referrals and with care coordination and health system navigation.
• Provide information for access and coordination of resources.
