Hand in Hand Behavioral Health Residential Facility Inc. is seeking an experienced HCBS Coordinator to oversee daily operations, supervise DSP staff, and ensure services align with Individual Support Plans and AZ DDD requirements.

This role blends remote administrative duties with periodic on-site visits across the Greater Phoenix Area. Applicants should have a strong knowledge of Therap, EVV, and electronic scheduling systems, along with proven leadership and communication skills to manage

Formación

• Experience in HCBS coordination or related supervisory roles.
• Strong knowledge of Arizona DDD policies and HCBS requirements.
• Familiarity with Therap, EVV, and electronic scheduling systems.
• Excellent leadership and organizational skills.

Responsabilidades

• Oversee daily HCBS operations including Habilitation, Attendant Care, and Respite services.
• Supervise Direct Support Professionals (DSPs); monitor performance and staffing.
• Manage staff scheduling, shift coverage, attendance, and call-offs.
• Monitor staff hours to ensure the 40-hour weekly scheduling policy is followed.
• Review Therap documentation, EVV records, service notes, and hours.
• Coordinate new member intakes, staffing assignments and ongoing services.
• Communicate with members, families, guardians, and DDD coordinators.
• Ensure services align with each member's ISP and are compliant.
• Monitor employee training and compliance requirements.
• Address member concerns and service interruptions promptly.
• Assist with DDD audits and corrective action plans.
• Provide regular updates to agency leadership.
