Job summary

Endeavor DDD Services seeks a Bilingual Care Coordinator in Phoenix to manage HCBS services for individuals with developmental disabilities. You will be the central contact between members, DSPs, and state support teams, ensuring service delivery aligns with PCSPs and regulatory standards, including AHCCCS and HIPAA.

This full-time role requires English/Spanish fluency, a fingerprint card, background check, driver license, and proficiency with Google Workspace.

Qualifications

• Fluent in English and Spanish (verbal and written).
• Fingerprint Clearance Card or eligibility to obtain.
• Ability to pass a criminal background check and drug test.
• Valid Driver's License, reliable personal vehicle, and insurance.
• Understanding of HIPAA confidentiality and compliance.
• Proficiency with Google Workspace and Apple/iOS devices.

Responsibilities

• Oversee HCBS for assigned DDD members across HCBS programs.
• Align care with authorizations and PCSPs; track outcomes.
• Conduct regular home visits to assess care and needs.
• Serve as liaison between families, DSPs, coordinators, and staff.
• Support DSPs; clarify goals and resolve delivery issues.
• Maintain compliant, accurate documentation per standards.
• Address gaps in coverage and advocate for resources.
