Job summary

Qualifications

• RN license in NM (or reciprocity) with 2 years direct clinical care experience.
• Growing or existing knowledge of wellness/managed care.
• Ability to communicate medical needs with members and staff.
• Willingness to travel within assigned territory.

Responsibilities

• Provide telephonic and field-based care coordination to inform and educate members on health programs.
• Engage members in discussions of adherence to personal health goals and respond to inquiries.
• Complete health screenings, assessments, and care planning, including home or facility visits.
• Support clinical operations with provider and member activities and coordinate with internal and external resources.
