Qualifications

• Active, valid nursing license in NM or equivalent licensure with authority to practice.
• 2+ years of clinical care experience and wellness/managed care exposure.
• Strong verbal and written communication; proficiency with Word, Excel, PowerPoint.

Responsibilities

• Provide telephonic and field-based care coordination for members.
• Educate members on health care programs and address health goals.
• Discuss medical needs with members and coordinate with providers.
• Conduct health screenings, assessments, and care planning.
• Travel to member homes/facilities as required for complex cases.
