UnitedHealth Group is seeking an RN Clinical Care Coordinator in Dedham, MA to manage a panel of members with complex medical and behavioral needs. You will coordinate care, develop person-centered plans, and collaborate with care teams to optimize outcomes.

Position requires MA RN licensure, 2+ years of clinical experience, and a commitment to patient education and safe transitions of care.

• 2+ years of clinical experience required.
• Current and unrestricted RN licensure in MA required.

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment.
• Develop and implement person-centered care plans addressing chronic health conditions and member safety.
• Partner and collaborate with internal care teams, providers, and community resources to implement care plans.
• Provide education and coaching to support member self-management of care needs.
• Support proactive discharge planning and coordinate care transitions after ER visits or admissions.
• Advocate for members and families to ensure their needs and choices are represented by the healthcare team.

About This Role

The RN Clinical Care Coordinator will be the primary care manager for a panel of members with complex medical and behavioral needs, focusing on care coordination to promote appropriate service utilization and improved quality of care.

Highlights

• Pay: $29 - $52/hour
• Location: Dedham, MA
• Schedule: Full Time, Day shift
• Travel: Local travel up to 75% with mileage reimbursement

What You'll Do

Requirements

Education

• High School Diploma/GED required

Experience

Licenses & Certifications

• Current and unrestricted independent licensure as a Registered Nurse in MA required
