Responsibilities

• Provides in-person and telephonic Medical Case Management to individuals and stakeholders.
• Discusses current treatment plans with physicians and explores alternatives.
• Assesses, plans, implements and evaluates patient progress.
• Evaluates treatment plans for medical necessity and cost-effectiveness.
• Attends doctors, providers, home visits and discharge planning conferences.
• Conducts home visits for initial evaluation and coordinates durable medical equipment.
