Job summary

Humana Inc. is seeking a Field Care Manager Behavioral Health to assess enrollees and connect them with clinical, community, and social resources across telephonic and in-person settings.

The role targets individuals with complex needs and requires coordination with an interdisciplinary team. Requires RN or LCSW licensure, Medicaid/Medicare experience, and strong Microsoft Office skills. Remote work with travel to SC sites.

Qualifications

• Active RN license or LCSW/LMSW/LPC/LMFT (or equivalent) to practice in SC.
• Minimum 1 year Behavioral Health clinical experience.
• 2+ years of case or care management experience.
• Experience coordinating services for Medicaid/Medicare enrollees.

Responsibilities

• Conduct telephonic and face-to-face assessments to evaluate enrollees' behavioral, physical, environmental, and psychosocial needs.
• Develop person-centered care plans and coordinate with clinical and community resources.
• Collaborate with providers and community-based services to promote outcomes.
• Travel to meet enrollees, families, and providers as needed (up to 50% local travel).
