Job summary

Humana Healthy Horizons seeks a Behavioral Health Case Manager in South Carolina to assess enrollees’ needs and coordinate clinical, community, and social resources for complex cases. This remote/field-based position requires travel, including to Humana’s Columbia offices, and reports to the Manager of Care Management Behavioral Health.

You will develop individualized care plans, collaborate with providers and community services, and monitor progress to promote optimal wellness.

Qualifications

• Active Behavioral Health clinical experience (1+ year).
• 2+ years of case or care management experience.
• Experience coordinating services for Medicaid/Medicare enrollees.
• Knowledge of community health and social service resources.
• Proficiency with Microsoft Office applications (Word, Excel, PowerPoint).
• Willingness to travel locally (e.g., to Humana Columbia office) for meetings and training.

Responsibilities

• Conduct telephonic and face-to-face assessments of enrollees’ behavioral, physical, environmental, and psychosocial needs.
• Develop individualized, person-centered care plans and guide enrollees to clinical, community, and social resources.
• Identify and resolve barriers to care and monitor progress across assessments.
• Coordinate with interdisciplinary care management teams to deliver holistic support.
• Determine appropriate in-person interventions and coordinate meetings as needed.
• Interpret complex information and align daily duties with organizational strategies.
