Job summary

Humana is seeking a Field Care Manager, Behavioral Health to assess and coordinate members' needs, guiding families toward appropriate resources to support care and wellbeing. This role involves telephonic and in-person assessments, care planning, and collaboration with a transdisciplinary team to improve outcomes.

Located in Louisiana with a remote work option, the role requires residing in Louisiana Region 7 and includes a 40-hour work week.

Qualifications

• Must reside in Louisiana Region 7 (Bienville, Bossier, Caddo, Claiborne, DeSoto, Natchitoches, Red River, Sabine, Webster)
• LCSW, LPC, LAC, LMFT or RN with minimum of 2 years of experience in psych.
• Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Responsibilities

• Perform telephonic and face to face assessments and evaluations to achieve/maintain optimal wellness
• Monitor progress and ensure desired outcomes through ongoing assessments
• Create member care plans aligned with departmental strategy and objectives
• Collaborate with providers and community services for quality and cost-effective outcomes
• Ensure delivery of Behavioral Health, Physical Health, Social Determinants of Health supports
• Coordinate across the transdisciplinary care team and transitions of care
• Submit incident reports
