Job summary

Humana seeks a Field Care Manager, Behavioral Health in Illinois to perform face-to-face and telephonic assessments. You will coordinate integrated BH and medical care, engage members in communities, and lead interdisciplinary care planning to improve outcomes.

Role requires travel up to 75% and working remotely with CST business hours. A licensed BH professional in IL and 2+ years of post-degree experience are required.

Qualifications

• Must reside in Illinois.
• Active Illinois license in LCSW, LMFT or LCPC (No supervisees or provisional licenses).
• 2+ years of post-degree clinical experience in behavioral health setting.
• Case management experience with complex SMI, SUD, SED populations.
• Ability to travel to region-based facilities and homes for assessments.
• Proficient with electronic medical records and MS Office tools.

Responsibilities

• Deliver high-quality behavioral health services through care coordination, crisis intervention, peer support, and collaboration with providers.
• Provide integrated support for members with BH conditions including SMI, SED, SUD.
• Engage members in communities with face-to-face visits to build trust and coordinate care.
• Coordinate BH and medical services, ensuring provider engagement and treatment adherence.
• Lead ICT meetings, oversee care planning, transitions, and service delivery.
• Support transition of care efforts and ensure smooth care handoffs.
