Qualifications

• Must reside in Michigan and be able to travel 15–20% to Wayne and Macomb counties.
• Bachelor’s degree in Social Work from an accredited institution.
• Active Michigan social work licensure (LLBSW, LBSW, LLMSW, LMSW, or LCSW).
• 2+ years of post-degree clinical or care management experience in behavioral health.
• Experience with adult populations, chronic conditions, and disease management.
• Knowledge of community health resources and social service agencies.
• Ability to travel to facilities and in-home for face-to-face assessments, including telephonic engagement.
• Valid driver’s license with reliable transportation.
• Strong communication and interpersonal skills; able to work independently.
• Proficiency with EMR and healthcare-related software; strong keyboard/web navigation.
• Experience with Microsoft Word, Outlook, and Excel.

Responsibilities

• Manage a caseload and complete assessments in home, community-based settings, or by phone.
• Provide clinical support and collaborate with Interdisciplinary Care Team for medically complex members.
• Develop and coordinate individualized care plans; ensure appropriate services.
• Identify barriers to care and advocate for optimal outcomes.
• Review and complete medical attestations with ongoing clinical oversight.
• Ensure services in least restrictive setting through ongoing needs assessment.
• Develop/modify Individual Care Plans with care team members.
• Support members/caregivers using an interdisciplinary approach to access resources.
• Serve as primary ICT contact, coordinating with members, care team, and external resources.
