Job summary

UnitedHealthcare is seeking a Field Care Coordinator to support a panel of members with chronic and complex health needs. You will work remotely from Idaho Falls, ID, engaging with members face-to-face or by telecommunication to create equitable, person-centered care plans.

The role emphasizes collaboration with internal teams and community resources, coordination of services, and education to empower member self-management. Travel up to 50% may be required depending on the case mix.

Qualifications

• Experience with Medicaid/Medicare or SDoH supports.
• Proficient in Word, Excel, and Outlook.
• Workspace secured for protecting PHI.
• Reliable internet access and ability to travel up to 50%.

Responsibilities

• Serve as primary care manager for a panel of members with chronic/complex needs.
• Conduct comprehensive needs/wellness assessments and create person-centered care plans.
• Collaborate with care team, providers and community resources to implement plans.
• Assist with HCBS supports and referrals as appropriate.
• Coordinate care transitions post-ER or SNF visits.
• Provide education and coaching for self-management and healthy lifestyle changes.
• Advocate for member preferences and coordinate nonclinical questions with health plan resources.
