Job summary

Humana Gold Plus Integrated is seeking a Field Care Manager Registered Nurse (RN) to assess member needs and guide families toward suitable resources for optimal wellness. The role includes creating care plans, coordinating with stakeholders, and visiting Medicaid members in homes and facilities across Illinois, with extensive local travel.

The position is hybrid with occasional onsite work. Requirements include IL RN license, 2+ years in-home case management, and strong computer skills.

Qualifications

• Must reside in the state of Illinois.
• 2+ years of in-home case/care management experience.
• Experience with adult population.
• Knowledge of community health and social services.
• Excellent communication and interpersonal skills.
• Ability to work with minimal supervision.
• Experience with EMR and Microsoft Office tools.

Responsibilities

• Identify and resolve barriers hindering care.
• Monitor patient care to ensure progress toward outcomes.
• Create plans of care for members.
• Communicate with internal and external stakeholders.
• Align with department and organizational strategy.
• Make decisions with limited direction and guidance.
• Follow established guidelines and procedures.
• Travel to visit Medicaid members in homes and facilities (75–90%).
