Job summary

UnitedHealth Group is seeking a Field Care Coordinator to manage a panel of members with low-to-medium complexity medical and behavioral needs. The role focuses on coordinating medical, behavioral, and social resources to promote appropriate service use and improved care quality.

This full-time, field-based position offers a home-based office with work from home when not in the field, and requires travel up to 50% within the assigned territory. Roswell, NM resident preferred.

Qualifications

• 5+ years of relevant experience, including 3+ years of clinical experience.
• Current, unrestricted licensure as RN or professional social worker with clinical background.
• 4+ years in a clinical environment (including 1+ years as a Care Coordinator).
• 1+ years of experience with MS Office, including Word, Excel, and Outlook.

Responsibilities

• Engage members to complete comprehensive needs assessments across medical, behavioral, functional, cultural, and socioeconomic domains.
• Develop and implement person-centered care plans addressing chronic conditions, wellness, and social determinants of health.
• Collaborate with care teams, providers, and community resources to execute care plans.
• Provide education and coaching to support self-management of health and appropriate lifestyle changes.
• Coordinate discharge planning and care transitions after ER visits or SNF admissions.
• Advocate for members to ensure their needs and choices are represented by the care team.
