Job summary

UnitedHealthcare is seeking an ECF CHOICES Support Coordinator to manage a panel of members with intellectual or developmental disabilities, focusing on medical, behavioral, and socioeconomic needs. This home-based role involves approximately 75% field responsibilities and requires commuting within a 50-mile radius of Hamblen County, TN, with optional remote work for eligible counties.

Qualifications

• Must have a degree in social work or a related human service field, nursing, or health care profession.
• RN or LPN certification/licensure acceptable.
• QDDP or QIDP certification if applicable.
• 5+ years as a separate HCBS waiver case management or independent support coordinator experience.

Responsibilities

• Engage members in needs assessments covering medical, behavioral, functional, cultural, and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions, health promotion, and social determinants of health.
• Collaborate with care teams, providers, and community resources to execute care plans and referrals.
• Coordinate internal and external referrals (e.g., Housing Navigator, Pharmacy Team, Peer Specialist).
