UnitedHealthcare is seeking a Field-based Care Coordinator to apply clinical knowledge to coordinate care for medically complex members, delivering self-management support and ensuring access to services and providers. This role emphasizes education, prevention, and cost-effective care across acute, home, chronic, and alternative settings.

Located in or near Davidson, Wilson, or Sumner Counties, TN, with remote work flexibility and strong travel up to 75% within the territory.

Qualifications

• Current Nursing License (RN)
• 2+ years of clinical experience as an RN
• 1+ years of experience with MS Office (Word, Excel, Outlook)
• Excellent organization, communication and time management skills
• Reside in or within commutable driving distance to Monroe County (TN)
• Access to reliable transportation and valid US driver's license with ability to travel up to 75% within an assigned territory

Responsabilités

• Apply clinical knowledge to effect efficient case management for Medicare/Medicaid beneficiaries
• Incorporate evidence-based practices into care coordination activities
• Provide member-centered, individualized self-management support and education
• Ensure appropriate utilization and consistent application of health benefits
• Conduct telephonic and face-to-face assessments per policy and timeframes
• Serve as member advocate to resolve barriers to care
• Coordinate community resource referrals for members and providers
• Develop and maintain an individualized care plan with team collaboration
• Monitor hospital encounters and provide education/interventions as needed
• Document and monitor all aspects of coordination in Health Plan software
