Job summary

UnitedHealthcare is seeking a Field Care Coordinator for Hamblen County, TN. This home-based role involves 75% field responsibilities and travel within the county and surrounding areas within a 50-mile radius.

As part of the ECf CHOICES team, you will assess needs, develop care plans, and coordinate services to support members with intellectual or developmental disabilities, while working remotely with flexible scheduling.

Qualifications

• Must meet one of the following: degree in social work, nursing, education or related human services; or be an RN/LPN; or hold QDDP/QIDP credentials.
• Experience as an independent Support Coordinator or case manager for 1915(c) HCBS Waiver with relevant trainings.
• Residence within 50 miles of Hamblen County, TN with ability to travel up to 75%.
• This role requires transportation and a valid US driver's license.

Responsibilities

• Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment across medical, behavioral, functional, cultural, and socioeconomic needs.
• Develop and implement person-centered care plans addressing chronic conditions, health promotion, social determinants, medication management, and safety.
• Create individualized care plans with goals and interventions aligned to readiness to change.
• Partner and collaborate with internal care teams, providers, and community resources to implement the plan.
• Provide referrals and linkage to appropriate internal or community-based services.
