Qualifications

  • Idaho license with one of the listed licensure options.
  • 1+ years of Medicaid/Medicare or SDoH-related experience.
  • 1+ years of MS Office experience (Word, Excel, Outlook).
  • Driver's license and ability to travel up to 50% of time.
  • Quiet home workspace with ability to secure PHI.
  • Residence in Idaho with access to high-speed internet.

Responsibilities

  • Serve as the primary care manager for dual eligible members.
  • Engage members face-to-face or by phone to complete needs/wellness assessments.
  • Collaborate with internal care teams and providers to implement personalized care plans.
  • Assist members with HCBS supports and services as needed.
  • Provide referrals and linkages to resources (internal or community-based).
  • Support discharge planning and care transitions after ER/SNF events.
  • Educate and coach for self-management of care needs and healthy lifestyle changes.
  • Advocate for member needs to ensure they are represented by the care team.
  • Assist providers with nonclinical questions and connect to Health Plan resources.
