Job summary

Reema Health in Gary, Indiana seeks a Community Health Navigator to engage members, assess needs, and guide them toward greater healthcare participation. This remote/field-based role involves weekly interactions, scheduling visits, and documenting work in the Reema App while coordinating with the Care Team.

You’ll manage a caseload, build community relationships, support referrals, and navigate social determinants of health.

Qualifications

• Solutions oriented: finds a path forward when barriers arise.
• Strong verbal and written communication.
• Tech-literate: quickly learn new tools on mobile and desktop.
• Self-motivated to stay productive on a remote caseload.
• Collaborative and able to engage in occasional in-person team meetings.

Responsibilities

• Conduct outreach by phone and in person to enroll members.
• Manage a caseload of 80-120 members and cover for others as needed.
• Assess needs and build member-centered care plans.
• Establish trusting relationships and follow through to completion.
