Job summary

UnitedHealth Group is seeking a Community Health Worker to serve members with complex medical or behavioral needs in Florida. You will engage community members, build care plans, and address social determinants of health to promote equity.

The role collaborates with a multidisciplinary team and requires field visits, flexible scheduling, and travel up to 10%. Ideal candidates have 3+ years in community health, 1+ year in case management, and knowledge of Medicare/Medicaid benefits.

Qualifications

• High School Diploma / GED
• 3+ years of experience working within the Community Health setting
• 2+ years of experience working within the local community, with knowledge of local resources
• 1+ years of case management experience
• Intermediate knowledge of Medicare and Medicaid benefits
• Access to reliable transportation and valid US driver’s licence for travel to client sites
• Ability to travel 10% of the time for role
• Set up a designated quiet home workspace with secure internet connection

Responsibilities

• Partner with clinical care management team to manage complex members in the community
• Locate and engage members in the community
• Conduct screenings to assess medical, behavioral, functional, cognitive and psychosocial needs
• Research and refer to community resources to address care or socioeconomic needs
• Support HEDIS/STAR gap closure with scheduling reminders and verifications
• Coach members for self-management and health promotion
• Collaborate with internal care team and community providers
• Assist in Medicaid benefits access and care coordination
