Job summary

CVS Health is seeking a Case Management Coordinator for OhioRISE in the West Region. The role is field-based with telework in Ohio and requires travel within the region up to 50% of the time.

Responsibilities include coordinating care plans, evaluating member needs, and ensuring quality of care across child-serving systems. Bachelor’s or Master in behavioral health or public health is required.

Qualifications

• 2+ years in behavioral health, social/human services.
• Experience with MS Office Suite.
• 2+ years in children's mental health or public sector human services.
• 2+ years in case management, child development, or related fields.
• 2+ years with Ohio delivery systems and local networks.

Responsibilities

• Manage complex cases in the community across child-serving systems.
• Coordinate care plans and monitor progress.
• Collaborate with care teams to address barriers and optimize outcomes.
• Serve as single point of contact for members and advocate for needed services.
